Use this form to make a payment towards your overall State fees.
Use this form to submit optional program book ad pages. If you are using this form, you will have to pay with a credit card while uploading (you cannot upload without paying). If you are paying with check, you must mail them in.
Use this form to submit optional photo entries. This includes the program book photo, top model, cover model, centerfold, and theme photo. Photogenic photos MUST be turned in at registration.
Use this form to order the official state tee. These are REQUIRED for ages 7 and up but are open to be purchased by anyone.
Use this form if you wish to fill out the contestant packet online. Everyone MUST turn in a copy of the printable registration form pages 2 (contestant information page) & 3 (optional categories chosen to enter in). These are used by the office, judges, and the emcee. Forms can also be mailed in if you do not want to submit online. If you wish to submit paperwork in the mail, you can find the paperwork on the Informational Sheets tab.
Use this form to submit A.L.I.C.E. essays online. This is for anyone aged 3-9 who is entering in this optional category. Ages 10+ will give a 3 min speech to judges at the pageant after interview.
Use this form to submit Kentucky Scholar online. This is for anyone who is entering in this optional category. This should be a one page of all school awards or achievements with the contestant name and age at the top.
